We support people of all ages living with complex health needs, long-term conditions, learning disabilities, and autism, helping them remain independent, confident, and connected to the life and community they value.

As a CQC-registered and digitally enabled provider, we use modern systems for care planning, communication, and quality assurance. This ensures responsive, transparent, and consistently high-standard support for individuals, families, and commissioners.
We champion Left Shift, Home-First, and prevention-first approaches, helping people stay healthier for longer and reducing avoidable crises. Our focus is proactive wellbeing, not reactive intervention.
SCL works in partnership with Neighbourhood & Integrated Care Teams, community nurses, therapists, PCN roles, social care professionals, and voluntary-sector partners to ensure coordinated, joined-up support that wraps around the person — not the system.

Signature Consortium was founded on a simple belief: that every person deserves care that is human, relational, and uniquely meaningful — care that reflects who they are, what matters to them, and how they want to live.
Our name was chosen with intention. A signature is personal, distinctive, and one-of-a-kind — and so is every individual we support. We created Signature Consortium to embody this truth: care should never be generic, rushed, or routine, but shaped around each person’s identity, story, and aspirations.
Our journey began with lived experience. Having supported our own loved ones, we saw first-hand how care could feel disjointed, inconsistent, and lacking in continuity — with too many people receiving help only when things reached a crisis point. We knew there had to be a better way: a model where people are supported to stay well, not just cared for when unwell.
So we set out to build something different — an organisation where continuity, connection, and coordinated support sit at the heart of every interaction, and where care is designed to promote health, prevent decline, and strengthen wellbeing from the outset.
Today, Signature Consortium is known for delivering inclusive, prevention-focused, and joined-up care that honours each person’s uniqueness and helps them live well for longer. We work collaboratively with individuals, families, communities, and multidisciplinary partners to provide support that is not only safe and reliable, but also personal, proactive, consistent, and truly aligned with each person’s life and goals.
